Service description:
The Early Help for Disabled Children’s Team is a part
of the Early Help and Prevention offer to families in
Northamptonshire.

The Early Help for Disabled Children’s Team work with
families with children aged 0–25 years, where the child
has a disability and the needs usually cover more than
one area of development and require support from two
or more agencies.

The team support the whole family to access
information and services which may be relevant to
the needs of their child.
We will assess and plan what is needed with you,
tackling the challenges faced by your family. We do this
through the Early Help Assessment (EHA) (formerly Common Assessment for Families (CAF)) which your family may already be working on or maybe something new to you.

What does the Early Help for Disabled Children’s Team offer?

The team will offer input to Educational Health and
Care meetings and Team around the Family meetings,
to provide advice and information about local disability
services.
When a case is allocated to the Early Help for
Disabled Children’s Team, the intervention will be
tailored according to the needs of the child and family.
The team will use the EHA process including the risk
and needs assessment to determine with the family
the nature of the intervention. A tiered approach will
be applied:
• Offer advice and guidance;
• Take the lead professional role and complete a EHA

What should I expect from the Early Help for Disabled Children’s Team?

The Early Help for Disabled Children’s Team will work
with your family for a short term period of support that
will require your commitment.
Your allocated worker may complete an assessment
with you, from which an action plan will be developed
alongside other workers supporting your family.
This is to ensure that the support you need is provided
in a co-ordinated manner. Alternatively they may
support the current EHA through advice and guidance.
The progress of the action plan will be led by your Lead
Professional and will be reviewed at ‘Team around the
Family’ (TAF) Meetings.
